Opioids are a class of powerful drugs used primarily as a treatment for pain and provide relief in the most excruciating of cases. Morphine, codeine, heroine, fentanyl, oxycodone (Oxycontin, Percocet) and hydrocodone (Vicodin) are all classified as opioids.<br />

<b>The High Risk of Addiction to Opioids</b><br />

Unfortunately, the risk of addiction with these drugs is high because opioids are so effective and can give users a feeling of euphoria. Short term use of these drugs is the safest option as it lowers the chances of developing a drug tolerance.<br />

Developing a tolerance means a patient must take higher and higher doses of the drug to experience the same euphoric effects. This deadly cycle of increasing drug amounts can destroy lives and lead to a potentially deadly overdose.<br />

<b>Treatment and Recovery from Opioid Addiction</b><br />

In cases where an opioid addict undergoes detoxification, withdrawal symptoms often make the process a terribly difficult experience. Signs and symptoms associated with withdrawal from opioids include: pain, intense emotional anguish, diarrhea, nausea, vomiting, hypertension, tachycardia and seizures.<br />

Treatment options for rehabilitation generally include a gradual reduction from the drug. Special drugs such as methadone and buprenorphine can help assuage withdrawal symptoms and reduce cravings. These drugs do not produce euphoric effects and have a lower risk of addiction.<br />

A great start to assisting patients during the rehabilitation process is the utilization of a simple and effective ear acupuncture protocol. The treatment consists of ten acupuncture needles applied to five points on each ear. After needle insertion, all a patient needs to do is sit quietly in a cozy chair for at least 20 minutes, up to one hour.<br />
Some patients may require additional acupuncture points. For example, a person manifesting signs and feelings of rage could benefit with the addition of an ear point called limbic system. It reduces aggressive, compulsive behavior which may lead to violence.<br />

This particular auricular therapy is known as the NADA protocol. Developed in the 1970's due to the efforts of Dr. Michael Smith at the Lincoln Memorial Hospital in New York, NADA stands for the National Acupuncture Detoxification Association. Dr. Smith created the NADA protocol as a treatment to help many suffering with heroin addiction in the local community.<br />

The success of the NADA program helped recovering addicts lower their chances of recidivism and increase their chances of entering society as healthy, drug-free individuals. In 1996, the World Health Organization declared acupuncture as a viable method to assist patients in withdrawing from chemical dependencies.<br />

Three reasons why acupuncture receives such high accolades for its success in treating opioid addictions, and other substances as well, is due to its simplicity, cost-effectiveness and absence of negative side effects. It is also safe for patients of virtually any age group, and in most cases, for pregnant women.<br />

Sometimes it is not only the patients who need treatment, but family members and loved ones who also suffer as their loved ones go through the withdrawal process. Application of the NADA protocol is safe for them and may help in dealing with their heavy emotions. It is support for the support team, so to speak.<br />

With an estimated 2.5 million people dealing with opioid addiction, safe and effective methods are needed more than ever to curb this national public health crisis. A recent 2017 White Paper "Acupuncture's Role in Solving the Opioid Epidemic: Evidence, Cost-Effectiveness, and Care Availability for Acupuncture as a Primary, Non-Pharmacologic Method for Pain Relief and Management" investigated acupuncture as part of a solution to the current opioid epidemic in the US.</div>
The paper provides evidence that acupuncture is effective for managing pain. This applies to pain from numerous medical conditions including migraine headache, low back pain, neck pain, nausea resulting from chemotherapy and others. The author referenced several scientific studies in order to reach this conclusion.<br />
<br />
There are two other important reasons to utilize acupuncture. One is the relatively low cost of the treatment, which can save money for patients and insurers. The other reason is the risk of negative side effects occurring is minimal, especially when compared with the adverse events associated with some medical procedures and pharmaceutical drugs. Acupuncture is a safe procedure when performed by a qualified practitioner.<br />
<br />
The paper reaches the conclusion that acupuncture is appropriate and useful as an adjunct therapy to treat and rehabilitate those contending with an opioid addiction. The FDA recently recommended in May of 2017 that non-pharmacologic options be explored before doctors prescribe extended-release and long-acting opioids, in an effort to reduce the problems associated with opioid dependence.<br />
<br />
Starting Jan. 1, 2018, hospitals will also provide non-pharmacologic options to help patients manage their pain. This mandate was set by the Joint Commission, a prominent, non-profit organization which certifies health care organizations in the United States. Acupuncture meets the requirements necessary to satisfy this mandate.<br />

Among veterans, musculoskeletal pain is one of the most common complaints, particularly low back pain (LBP). The number of veterans reporting to the Veteran’s Health Administration (VHA) with low back pain rose an average of 4.8 percent per year between 2000 and 2007, exceeding the rise in rates of diabetes, high blood pressure and depression.<br />
<br />
A survey of 15,000 veterans from the Persian Gulf War found that 45 percent reported experiencing back pain. Chiropractic services have been added to the care options for veterans, and most chiropractors providing services to veterans within the VHA reported that LBP was the primary complaint.<br />
<br />
A study to determine the effectiveness of chiropractic care in treating veterans with LBP was performed on 171 patients who met the inclusion criteria. The average veteran was male, obese and suffering from chronic LBP (for over 6 months, on average). Most participants also presented with other physical and psychological issues.<br />
<br />
Post-Traumatic Stress Disorder (PTSD) is common in the veteran population and has been shown to contribute to chronic pain.<br />
<br />
Veterans in the study received treatment from a chiropractor in the western New York area after being referred by their primary care physician. The number of treatments given to each case during the course of the study ranged from 2 to 26, with 8.7 being the mean number of treatments per person. Treatments were performed twice a week on average, and the patient was assessed after every four treatments.<br />
<br />
Chiropractors used a range of treatment modalities on the subjects, including standard adjustments, spinal mobilization and flexion distraction. Patients were also instructed on how to perform therapeutic exercises and do stretches that were tailored to their specific condition.<br />
<br />
Pain severity was measured by the Numeric Rating Scale (NRS), which is a verbal rating by the patient on the severity of pain using a scale ranging from 0 to 10 at the time treatment is undertaken.<br />
<br />
The Back Bournemouth Questionnaire (BBQ) is a biopsychosocial measurement of low back pain symptoms consisting of 7 questions that measure pain, disability, and the affective and cognitive-behavioral aspects of musculoskeletal problems. Scores on the BBQ range from 0 to 70, with higher numbers indicating a higher severity of symptoms.<br />
<br />
After only two chiropractic treatments, 60 percent of veterans showed a significant improvement of 37.4 points on the NRS and 34.6 points on the BBQ measurements.<br />
<br />
he researchers concluded that, “Despite high levels of service-connected disability and comorbidity, veterans' chiropractic clinical outcomes in terms of mean percentage improvement from baseline to discharge for both NRS and BBQ were statistically significant and clinically meaningful.<br />

This study adds to the understanding of chiropractic clinical outcomes for veterans with LBP and contributes to a foundation for further research.”<br />

Researchers at University of California, San Francisco set out to determine if there is a link between vitamin D status and developing new lesions in patients with relapsing multiple sclerosis (MS).<br />
<br />
The researchers used data from a 5 year longitudinal study of MS patients. The participants have clinical evaluations, a brain MRI, as well as blood work annually.<br />
<br />
The measured 25(OH)D levels were assessed for an association with T2 lesions on brain MRI, clinical relapses, and disability. T2 images from an MRI show both old and new inflammation on the brain since onset of MS.<br />
<br />
The authors collected a total of 2,362 brain MRI scans from 469 patients. The researchers found that with each 10 ng/ml increase in 25(OH)D level, the risk of new T2 lesions in the brain decreased by 15%. Also, the 10 ng/ml increase was associated with a 32% lower risk of gadolinium-enhancing lesions, representing “active” lesions. They also found that higher vitamin D levels were associated with lower relapse risk, although this finding was not statistically significant (p=.037).<br />
<br />
The authors encourage future randomized trials looking into the relationship between vitamin D and brain lesions in MS.<br />

<b><u>RCT: Vitamin D supplementation in patients with MS</u></b><br />

Dr. Soilu-Hänninen and colleagues from the University of Turku in Finland recently conducted a double blind, placebo-controlled trial of vitamin D in 66 MS patients, to see if it helped as an add-on therapy with interferon.<br />

They found that 20,000 IU of vitamin D3 once a week resulted in significantly fewer lesions on brain MRI and strong trends toward lower lesion burden, reduced disability scales, and improved ability to walk, compared to controls. 25(OH)D levels only went from 22 ng/ml to 44 ng/ml in one year.<br />
<br />
The authors note that other studies have found that in children with possible MS, each 4 ng/ml decrease in 25(OH)D has been correlated with a conversion to definite MS. Lower serum 25(OH)D values have been associated with a higher rate of MS relapses. Researchers have correlated each 4 ng/ml increase in 25(OH)D with up to 12% reduction in relapse rate in adults with MS, and each 10 ng/ml increase in 25(OH)D with a 34% decrease in relapse rate in childhood MS.<br />
<br />
Remember, observational studies (unlike the RCT above) are likely studying the effect of sunshine, not supplements, and the 25(OH)D levels are more likely to be a reflection of how much sun exposure a subject gets than whether they are taking a supplement or not.<br />
<br />
In spite of the above findings in a randomized controlled trial, I would never recommend only oral vitamin D for people with MS. They may well benefit from sunshine or low-pressure sun tanning beds as well.<br />
<br />
Until it is clear oral vitamin D can fully replace sunshine, make sure all MS patients take at least 5,000 IU of vitamin D per day and sun bathe daily at midday. In the winter, use low-pressure sunbeds, the ones that replicate high intensity sunlight, on a regular basis.<br />

Bisphenol A (BPA) is an organic chemical used in the manufacture of polycarbonate plastics and epoxy resins. It is found in many hard plastic drink bottles and metal food and drink cans. Although it has been in commercial use since the 1960s, it is only recently that concerns have been raised over its use in food containers. These concerns are based on the fact that BPA has been observed to have a detectable hormonal action on the body, binding to estrogen and other endocrine receptors, a finding that has prompted several governments to reassess the safety information on this substance.
A study in 1983 by the United States National Academy of Sciences concluded that exposure to BPA presented a minimal hazard to human health. However, in 2000, the US National Toxicity Program carried out a review of the BPA safety evidence, particularly in relation to its action as a hormonal disruptor at low doses, and suggested that “There is credible evidence that low doses of BPA can cause effects on specific endpoints.” The review panel did go on to say, however, that the weight of evidence was not conclusive and there was uncertainty as to the “biological relevance” of the findings. Nevertheless, two areas of recent research have disputed the apparent safety of BPA.
Most of the previous studies on BPA used rats and other lab animals, but recent studies have been done that look directly at the effect of the chemical on humans; in particular, a ten year study carried out by scientists from a number of institutions, including the University of Exeter and European Center for the Environment and Human Health, clearly demonstrated a link between BPA exposure and the risk of developing heart disease1. Further, and perhaps even more alarming, a study by a team of American pediatricians found that higher BPA levels in infants both gestationally and after birth were associated with a lack of behavioral control2. This effect was greater in female infants, as might be expected, since BPA can disrupt healthy estrogen function.
Alone, this research would be enough to give cause for thought and definitely for concern, but it comes on the heels of other studies which have pointed to the potentially hazardous nature of BPA to the human body 3,4,5,6,7. The Canadian government announced in 2010 that it considered BPA to be a toxic substance unsuitable for human consumption, and the European Union, United States and Canada have since banned the use of BPA in baby bottles.
Given the ubiquity of BPA in the food industry, it is unlikely that researchers are going to be content to stop here, and further findings in the near future are to be expected. Indeed, a follow-up study on the link between exposure to BPA and the development of heart disease has already been published8. Although the industrial production of bisphenol A is worth billions to the world economy, governments have already demonstrated that they are willing to tighten regulations on the usage of BPA, and, if there is more evidence of the toxicity of BPA to human health, further measures are entirely possible.

Most large cities continue to get larger.<br /><br />The takeaway relates to the methods and means we can employ to counter the effects of living in the big city. These often deleterious effects are well-known and yet worth repeating. Urban air quality is notoriously poor. We all can easily conjure up a mind's-eye view of the oily ochre tint of many metropolitan skies. Food in many urban areas has lost much of its nutritional value owing to the great distances meat, dairy products, and fruits and vegetables have to travel to get to the city supermarkets and marketplaces. Water quality is often degraded by nearby industry and sewage treatment plants. Green space is at a premium in most urban environments - there are few places in which to play outdoors.<br /><br />How can we combat these tradeoffs in air quality, water quality, food quality, and lack of nurturing green space? First, it's important to recognize that these tradeoffs exist. The human organism was not designed to live in crowded cities. Our bodies were designed to thrive in a richly diverse outdoors environment, sowing and reaping in various ways and participating in complex ecosystems. <br /><br />Now most of that is gone and we pay the price in terms of lifestyle diseases - diabetes, obesity, and cardiovascular disorders.1,2,3 The good news is that there are many action steps we can take to restore precious health to ourselves and our loved ones. <br /><br />Two specific actions focus on food quality and outdoor activity. First, whenever possible, buy food produced locally. In general and all things being equal, the less distance food has to travel to get to the dining table the more nutritious it is. All that's required is to find out where the local farmers are setting-up their stands.<br /><br />The other specific action involves engaging in outdoor activities several times per week. Being in the presence of sunlight, trees, flowers, birds and small wildlife, and shrubs, plants, and ground cover provides nourishment that is not measured in calories. However, in most cases, it should get better on its own, within six-eight weeks and there are certain things that you can do and avoid doing to aid and speed up your recovery.<br /><br /><span style="font-weight:bold;">What happens in a slipped disc?</span><br /><br />Although people often mention about having a slipped disc, nothing in your spine has actually slipped out of place. Having a slipped disc means that one of the discs which sit between each of the bones in your spine has been damaged. When the disc is damaged, the soft gel-like inner pad of the disc squeezes out through a weak point in the torn outer layer, causing a bulge that often presses on nearby spinal nerves. This result in severe pain with symptoms that radiates down the leg/arm commonly referred to as sciatica. Slipped disc, also known as disc herniation, can occur in any disc in the spine but the two most common forms are lumbar disc and cervical disc herniation.<br /><br /><span style="font-weight:bold;">What can I do?</span><br /><br />During the first 48 hours, a torn outer layer of the disc would result in the release of inflammatory chemical mediators which may directly cause severe pain, even in the absence of spinal nerve compression. This is the basis for the use of Ice & natural anti-inflammatory supplementation. Avoid standing or sitting (e.g. driving) for extended period of time as it would increase strain to your spine and aggravate disc pain. At home, keep away from overstuffed and low furniture, because it is difficult to stand back up after sitting in them. Don’t lie on stomach and prolonged bed rest especially during early stage post injury.<br /><br /><span style="font-weight:bold;">In the long run<br /></span><br />Back pain from a slipped disc may return, whether or not you have had treatment and it is important to learn how to avoid damaging your back again.<br /><br />The outcome for most people is that they will feel better within six-eight weeks; although for others it may take a while longer. Many of these have better services than my insurance has. <br /><br />Mr President: It is the small business owner that is suffering, we can't provide for employees because we have insurance agents selling one company (usually Anthem Blue Cross) & not working for us. Millions could be saved with high deductible plans with both employer, employee & maybe even small Tax to pay for the deductible portion cutting standard premiums in half. The problem is individuals do not get the same treatment as large groups do, premium increases & health issues are an ongoing problem and are penalized to the point of not being able to pay for the increased cost of insurance. Also Cobra is ridiculous, you don't get the Employer discount and often the rate is not affordable to keep.<br /><br />We need a uniform State By State system with uniform pricing & ratings for the entire population. Remove any barriers to competition & reward people for staying healthy!<br /><br />If life insurance companies can give a set rate for 20 years, why can't insurance premiums be set? <br /><br />Eliminate Medicare fraud period. Millions of dollars are spent on treatments that offer no value and cost patients & taxpayers millions, I am specifically talking about cancer treatment in this country but that's another story...<br /><br />Our society will not be healthier because we all have insurance, we are fatter, sicker, & more stressed/depressed and yet we have access to the "best" medical care in the world. Stretching prepares you for the work of exercise, and is done first — before anything else — gently and gradually.<br /><br />You may be tighter than usual on a particular day. This is not important — you should never try to stretch to where “you think you should be”. Just stretch, making sure to pay attention to what you’re doing. It’s easy to injure a muscle if you’re thinking about something else, or if you’re rushing, trying to squeeze in some stretching before dashing off to the gym.<br />You never know until you hurt it how much you use your lower back all day long. When your lower back is injured, every movement becomes painful. Simple actions, such as getting out of a chair or bending over the sink, become excruciating, and your daily routine becomes difficult and frustrating.<br /><br />Back pain affects 60 to 80 percent of U.S. adults at some time during their lives, and up to 50 percent have back pain within a given year. Some of these problems are easily treated and never return, but in five to ten percent of patients low back pain becomes chronic and the person continues to have recurrences and exacerbations.<br /><br />Effective treatment of uncomplicated lower back pain involves treatment in a chiropractor’s office and beginning and continuing an exercise program. A recent study conducted by the Medical Research Council, a research organization based in the United Kingdom, has found that patients given a combination of spinal manipulation and exercise experienced greater improvement in back function and greater reduction in pain compared to those treated with spinal manipulation or exercise only.<br /><br />Most mechanical lower back pain is associated with tight leg muscles and weak abdominal muscles. Leg muscles need to be stretched and abdominal muscles need to be strengthened to avoid recurrences of lower back pain.<br />People are generally not aware of these relationships. You may know you “should be exercising”, but you may be unaware of the importance of stretching. Also, abdominal strengthening is usually the last thing a person thinks of when he or she thinks of doing exercise.<br /><br />Abdominal strengthening helps support the lower back. Spinal muscles are not designed to carry your body weight. If your abdominal muscles are weak, then your back muscles will be used to carry your body weight, and eventually you’ll have a lower back injury. Abdominal strengthening not only helps keep your lower back healthy, but also helps maintain good posture. Postural benefits include an easy, relaxed gait; muscles that are long and supple, rather than short and tight; and an open chest that allows for easy, smooth breathing. Your body is a machine. Everything’s connected. A lower back problem affects many other areas, ultimately. Think About This For 2010<br /><br />This past Sunday, I attended the funeral of a friend. She died after six months of cervical cancer that did not respond to traditional and alternative treatments. She was a happy soul, full of smiles for her family including three grandkids. As I stood with others at the gravesite, my emotions began to rise with each eulogy and picturing the gatherings we attended together. Life is too short and too bitter at times.<br /><br />She was the last I would have thought to be stricken and taken away prematurely. I imagine many of you reading this have similar tales and sorrows and for that I am sorry. I write this two days before the New Year as a reminder that there is much work left for us to do both personally and professionally and is the perfect time to resolve them seriously and get our acts together.<br /><br />Doctors, we need you, I need you. Look in the mirror and what do you see? Are you overweight, possibly physically fragile and challenged because you neglect yourself? Has your previous meals been tasty but void of nourishment, full of enhancements and noxious chemistry? Is your time spent stressed to perfection or depletion? You get my drift, this year the typical resolutions of losing weight, reading more books, taking extended vacations, exercising more should take a greater prominence because we are all marked toward extinction. The how and when to some degree you may control. You are worthless as a healer, a teacher and role model if you are unable to sustain a working day. You are reducing your self worth if you deny yourself the pleasures of health and look back at what coulda and shoulda been. Doctor, so many people beyond your family depend on you and no one more that we, your colleagues, your patients and staff.<br /><br />Besides your personal challenges, you have professional obligations. How many people in your community can you literally save and how many have you yet to meet. This is not a stretch as timely reminders of health and wellness can turn a sickness and disease process into health and wellness. Once you have an opportunity to meet them at work, their social gatherings or sports venues you have an opportunity to share. If they take the proactive step to visit your office and be evaluated you have a greater opportunity to review and discuss and if they become a patient and refer their family and friends your impact can be quite profound. Add rehab exercises, adjusted daily living advice and working with them ergonomically you did save a life or add more quality to their existence. <br /><br />These are no mere words. I recommend you attend a community funeral and see and hear those left behind. From physio to sociological the body will perform both at ease and in better harmony with less neurological and physical stress. This we should not debate. Friends, this is our department and where I’d like to leave my final thoughts with you in 2009. Get ready for the storm of success by gearing up with the tools that will get you there. A strong and committed staff, a well worked promotional department and patients understanding the real healthcare crisis, that is a lack of health care. We are in the forefront and the battleground and it starts right in your office and down th street in your community. In a year of doom and Gloom the residents of Fluvanna pulled together to provide at need children a wonderful holiday season! Locally we collected in one day over 500 New toys & bikes and $2305 in cash! This has far surpassed previous totals. many were $5.00 , $10.00 donations and several large ones. All of these donations are pooled together with Charlottesville and local school districts go shopping for children at need. According to Toy Lift over 3000 children will have a brighter Holiday because of the generosity of many of you! Preliminary results for Charlottesville is over 11,000 toys & 4000 books collected.<br /><br />I could never pull this off by myself and the cast of characters & contributions is large. There have been many question and concerns regarding this and I think it is good for us as parents to question the safety and efficacy of what we do for our kids.<br /><br />Personally I have chosen not to have my children immunized, This after many months of research and information that I have reviewed.<br /><br />I have stated on my Facebook page at Lauterbach Chiropractic and blog for many months about how prevention is the key to staying healthy. <br /><br />Children should be getting proper rest, avoiding sugar, washing hands and most importantly if you child is sick do not let them go to school and do not let them go back to early as they will be prone to secondary infections.<br /><br />I have done a lot of research on Vitamin D: the recommended dosage for children is only 400 units, My children take 1500 - 2000 day, especially now. You can google Vitamin D council for more info.<br /><br />Vitamin c, zinc, echinachea (root) are all good and you should invest in good quality brands with little or no sugar added. If a child has signs of the Flu natural elderberry has been shown to be more effective than Tamiflu.<br /><br />It is tragic when children die, most anyone that has died from H1N1 has had some type of immuno-compromising disease such as: cerebral palsy, MS, etc.<br /><br />In a recent interview with Dr. Oz (you can google that to) it was on CNN. Dr. Oz went on to say that he had to get the shot because of mandates from his Job, but his wife and children would not be getting it.... Unfortunately, the state epidemiologist was not interested in studying it further so I pass it on to you since I think it is noteworthy.<br /> <br />CWC is a long-term care facility for people with developmental disabilities, home for approx. 275 people with approx. 800 staff. Serum 25-OHD has been monitored in virtually all residents for several years and patients supplemented with vitamin D.<br /> <br />In June, 2009, at the time of the well-publicized Wisconsin spike in H1N1 cases, two residents developed influenza-like illness (ILI) and had positive tests for H1N1: one was a long-term resident; the other, a child, was transferred to us with what was later proven to be H1N1.<br /> <br />On the other hand, 60 staff members developed ILI or were documented to have H1N1: of 17 tested for ILI, eight were positive. An additional 43 staff members called in sick with ILI. (Approx. 11-12 staff developed ILI after working on the unit where the child was given care, several of whom had positive H1N1 tests.)<br /> <br />So, it is rather remarkable that only two residents of 275 developed ILI, one of which did not develop it here, while 103 of 800 staff members had ILI. It appears vitamin D is incredibly protective against H1N1. Dr. Carlos Carmago at Mass General ran the numbers in an email to me. Even if one excludes 43 staff members who called in sick with influenza, 0.73% of residents were affected, as compared to 7.5% of staff. This 10-fold difference was statistically significant (P<0.001). That is, the chance that this was a chance occurrence is one less than one in a thousand.<br /> <br />Second, if you read my last newsletter, you will see that children with neurological impairments, like the patients at your hospital, have accounted for 2/3 of the childhood deaths for H1N1 so far in the USA. That is, the CDC knows, because they reported it, that patients with neurological impairments are more likely to die from H1N1.<br /> <br />The problem is that I cannot get anyone in authority at the CDC or the NIH to listen. I need readers to email or call their senators and congresspersons in Washington.<br /> <br />Ask your senator or congressperson to contact the CDC and NIH to complain about CDC and NIH inaction on Vitamin D and H1N1.
